You already pushed your luck as far as you can. In fact further than any other employer would have allowed, so count your blessings and DO NOT ask for more time off before you start work. You're just going to have to go straight home after work and stay up late, or get up early in the morning, study at lunch, etc. You need to set priorities and you can't expect others to alter their needs to suit you. You aren't the only one they met with. When you're talking about July to October, you didn't use the time wisely.

You need to be honest with yourself and your new boss. When are you scheduled to take the exam? If not, have you given your self or your new boss an estimate on that date? You may suggest part time work until you take the test to give you time to study and learn the ropes and the new firm. Ultimately this is up to your new boss if this is acceptable . You can bring the suggestion to the table about an extension or part time work and if it is not acceptable perhaps you can work to together to make it work. Could you work 4 ten hours days and have three days off to study? Or do you need to do it all at the same time in order to have a job? If I were the boss my question would be why you need the extension. Is the material to hard to understand or did you not stick to your study pattern or perhaps there was a lot more to it than you realized. Think long and hard about it but you need to talk to them. Make a list and pull yourself out of the situation. Play like you are the boss and put yourself in a worst case situation. Are you comfortable or not? What can you do to make everyone happy?

find a new way to study for the exam. I wouldn't ask for another extension, they could just hire another person that has already taken the exam. I'd suggest you cancel ALL your plans, get off the computer and start studying day and night. Or kiss your job goodbye.

I am totally serious here. Suck it up, pull longer hours, get the studying done. Once extension is reasonable. Two is a red flag... You probably wouldn't get fired but you'll always be that person who couldn't "get it done".
Get it done. Pass the CPA & start work in October.

Your integrity and honesty will not go un-notice if you sit down and engage the person responsible for for hiring you and explain your situation as a future advantage for the company. If the company is reputable they will access your proposal and realize that ultimately you are moving them to the next level.
Some issues you may want to evaluate or re-evaluate are: You made a commitment (assumed) and the company originally expected you to follow through and now you're not... Does that bode well for you? Probably not, unless you make it a taking point when you ask them what "they think" you should do about this situation.
All said and done, you are the one asking for a favor. You need to insure them it's to their advantage.

You need to really explain the situation to the company that has hired you. It could well be that you can work less hours to start with giving you time to study or indeed that they are entirely favourable to the extra extension.
Before you panic you need to talk to them and see what the options are.

Join for duty, then take official leaves of absence periodically, and complement it by burning some midnight oil. Also take some time off your rest days and holidays for additional study. Keep your mind relaxed, confident of qualifying with flying colours.

Heard of Multi-tasking..you gonna need it in life. You will never be able to hold off one thing to do another if you start off like that. you have to juggle work, family, relationships, phone calls, shopping all at the same time....
They need their work done too, have you thought of the backlog of work that they need for someone to do? it's not about you sweetie.

Unfortunatley i think you just need to study hard and work, its the reality of life nothing is easy and you shouldn't blow yur chances with this job. something tells me yur a bit of a procrastinator and if thats the case, well you reap what you sew.
