Prepare a dossier of affidavits that you do not smoke. Prepare to sue them in small claims court. If you paid with Amex, get them to support you (other credit card firms will generally leave you on your own, but Amex is usually helpful.
Complain to the Chairman of the hotel chain. Write to the Better Business Bureau. Write to the travel columns of newspapers and to travel magazines. Always enclosing a copy of your doctor's letter attesting to the fact you never smoked.
Eventually, if you persist, you should get results.

No, as it is your word against theirs and unless there has been a pattern of "false reporting" by the manager of the hotel in the past, why would there be any reason for them to lie about the situation? Even if you get a good private investigator, your have not shown any real damages other than being charged a higher rate for the room for smoking and your fees willl be higher than what you would ever be entitled to receive (the difference between what can be charged for a room that allows smoking and one that does not, or any additional fees that you are charged if the entire establishment is non-smoking). Unless there has been a demonstrated pattern, there is no reason for a hotel manager to lie against you as opposed to other tenants. Either suck it up and move on, or be prepared to spend a vast amount of money for a Small Claims Court suit that you have to prove by the preponderance of the evidence of ANY state you reside in.

Did you invite someone in who did smoke, you are still liable. Do they have proof someone smoked in your room? Stop the credit card or cheque and let them take you to court if they disagree.

Do what Andy says and then speak with your family doctor a.s.a.p. you may be able to prove your case with a simple blood or urine test and even possably a hair sample test.
Best of luck.

Did you pay it already?! Ask them WHY?! I'd talk to the supervisor of the hotel..and threaten them with taking them to the better business agency and stop payment on whatever you paid them with...
