I need to provide a receipt to someone for services rendered and would like know what information I need to put on the receipt.

Your name (or business name), phone number, customer name, type of work done and itemized cost of product and labor with total at the bottom. Better to give too much info that not enough. Besides, if they want your services again, they will know how to reach you or give your info for a referal.

A basic receipt should include your name or business name, the date, an itemized list of products/services provided and the cost for each, tax charged, the total cost, and the customer's name.

wait a minute if they provided you a service then they are not a customer. they provide you with a bill , you pay it and he marks it paid that is his receipt. he then gives you a copy so that you have proof that you paid it. no harm mate but that is kinda basic

All that's really required is your business name and a total but you can get as detailed as you want and the customer requests.
