it seems that the more a company tries to eliminate mistakes. by adding more and more rules seems to create more and more costly mistakes. is that true? if so why? and if not why not?

Just trying every loophole to cover their asses...But of course the higher the numbers get's with a companies growth, the larger and more frequent the mistakes seem. It's a numbers thing...

Usually because they are overloaded with work and are in hurry to get things done. When the boss piles work on your desk and you face a looming deadline, you rush through it. In other words, haste makes waste.
