They can help change where the image focuses in the eye. The lens of the eye causes the light beams reflected from the image you look at to focus at a certain area. Some people have the image focus too soon, so the image would be clearest in front of the retina. Some people have a lens that would cause the image to be sharpest too far back.
Glasses help focus the image at the retina, which is at the back of the eye. The retina is the part of the eye that processes images, and the optic nerve connects with the retina to send signals to the brain.
If you have a physics textbook, look up concave and convex lenses and you will see what I mean about light beams being focused at a certain area.

No, wearing glasses will not fix your vision. It may help slow down their deterioraton. People who wear contact lenses have been known to have their vision improve slightly due to the gentle pressure on the eye. This is only true with certain types of vision problems.
