Acne is a condition of the sebaceous hair follicle. The hair follicles, or pores, in your skin contain sebaceous (oil) glands. These glands make sebum, which is an oil that lubricates hair and skin. Under normal conditions, sebum travels up the hair follicle and out to the skin's surface. However, with acne, sebum is trapped within the follicle, clogging the follicle. The bacterium that seems to cause acne is called Propioibacterium acnes (P. acne) and is present in everybody's skin, usually lodged in the hair follicle. The clogged follicle becomes a breeding ground for bacteria. When sebaceous glands get clogged, sebum builds inside the follicle; P. acne multiplies rapidly and can result in acne.
P acne bacteria thrives in an oxygen free environment and feeds on the trapped oil in the sebaceous follicle. It then releases a toxic, corrosive fatty acid by-product that works to break down the follicle walls and encourage the leaks and ruptures that produce acne lesions.

The root cause of acne is normally genetic, though it is certainly true that things like dirt, oil, clogged pores in general, and even foods can aggravate the condition. My derm told me that foods dont cause acne, its only genetics, but I have done controlled experiments with chocolate, and believe me, when I do eat chocolate which is maybe 4 times a year, I break out really bad. Of course this varies from person to person. You have to find out the specific things that trigger acne for you. In general, when I avoid foods high in sugar and grease, I dont really break out, so my dermatologist is an idiot!

Anything that happens on the outside of your body leads back to problems internally. For acne, it has to do with your liver and hormones. Your liver can't bear the load of hormones and so it causes acne. Also, sweat is a huge cause of acne.

Having your skin dry out and get dirty causes acne. As your skin cracks, bacteria and dirt get trapped inside. That is why it is important for people with acne to wash their faces, use creams that cause them to exfoliate, and use mosturizing lotion.
