For example if a person answers, "Fine" is that person really fine?

Never it is just a social question and no one ever expects the other person to really say how they are...but that is in a purely everyday passing circumstance. If it is a close friend or family member concerned about your welfare and something traumatic has just happened it may be a different story...depends on who is asking and under what circumstances.

When people ask that, they're not usually really wanting to know how the person is doing unless they're a doctor or something. It's a formality...."Hi, how are you doing?" It's more of a greeting than it is a question. Most people asnwer "fine" because it's the easiest. "Good" is another common response.

doesn't really say how they feel.
people ask and answer the question the same as 2 saying hi.

Rarely. Most of the time you just get the same old perfunctory answer:fine--even though your world might be falling apart.

no it is a universal ice breaker and every one wants to think your doing fine......test this by telling them your not OK and start describing your troubles,,,,see how fast they move on.
