The factory heatsink should have a patch of thermal grease on it. However, you can clean this off with some contact cleaner. I would then get some "Arctic Silver" thermal grease and put a BB sized drop on the center of the processor core and then install the heatsink.

Depends on how much is on there. You say alittle....that means to me that its a very thin amount on the processor. I would might go ahead and stick alittle more if I was building my system. Be careful not to put thermal grease over the whole cpu just alittle in the very middle only. Too much over the processor will have a insulating affect......will just get hotter. While having just alittle helps prevent cpu from frying and help it dissipate heat faster.

make sure you cpu has thermal paste, unless you want your cpu to burn up, as long as the one from the factory covers the whole area then it will be fine

leave it as it is if you plan on doing normal stuff. i am using one now with stock cooler and its doing just fine. if you plan to overclock, then do so but install a different cooler with much better cooling efficiency.

Those processors run really hot. Make sure that you have enough air flow in your case. Also your air-flow should run from the top to the bottom.

if you haven't modified it then no but if you have then scrape off the old and add some but not too much.
