Are you exfoliating? I'd love to help you out. No catch, I just love my "job."
It sounds like maybe you don't get enough exfoliation or moisturizer, or both! You may need to retexturze your skin. Avon has two good products for that - Anew Clinical Microdermabrasion, and Anew Clinical One-Step Chemical Peel. I'd need to talk to you, do a little Q&A to find out what's going on.
That said, to apply foundation, begin in the center of the face and blend out and down in circular motions until blended. Do NOT go past the jawline.

If you are getting flaky in some spots it seems you need to put moisturizer on your face before you apply your foundation. A clean, non-oily moisturizer will help all make-up blend in better. Dependign on how much money you want to spend depends on the type of makeup to buy. For a decent price and for shopping at Target/Wal-Mart, etc try Loreal True Blend. The color is natural and blends realllllly well. If you are willing spend more, try MAC cosmetics usually found at your local Macys (you can look online at their website for exact locations). Sponges for applying do work well, but I perfer my fingers. Eyeshadows and blushes all seem the same now days. Cream shadows and blushes look more natural and you can blend them into a shade you pefer. Always go with a concealer a few shades lighter then that of your normal foundation color. Physicians Formula has these great colored sticks that hide reds from blemishes and under-eye circles that are dark. I love them and swear by them!

You want to start on the top of your face (your forehead) and work your way down with your foundation. Believe it or not, your skin cells on your face are like shingles. This means that if you apply your makeup going up, the makeup gets caught underneath your skin cells thereby clogging your pores and making you more prone to breakouts. With your eyeshadow, you want to stick to applying it just on your eyelids and not any further or it will look goofy. With your blush, smile really big with your mouth closed and apply from your cheek stroking upward toward your ear. With your concealer, you want to try to apply it with a small makeup brush. Just dabbing on your concealer wtih your finger makes the blemish stand out more because now you have a big blotch of concealer over it. A makeup brush will apply it smoothly. I hope I could help :]

make sure you apply foundation to clean, toned, moisturized skin. all these steps are necessary. after moisturizing try using a primer, i prefer smashbox photo finish with spf, next rub a little concealer on the back of your hands inbetween your thumb & index finger to warm it up.

gently apply the concealer where needed, usually under the eye area &/or scars on the face. next is the foundation itself, make sure you use a brush & the correct one when doing this. try using a buffer brush instead of a foundation brush. that will give you the barely there airbrushed flawless skin look.

foundation wise i personally prefer bare minerals & brush wise i use MAC #187. if foundation is a little too heavy for your liking try a tinted moisturizer. i use laura mercier or stila, both are great.
