Download Everest Home Edition or Belarc Advisor, it will query your system and provide it

In order to find that info out, you will need to go into your BIOS.
When booting up your PC, hit "delete" or "F2" (the screen should tell you which) to get into BIOS.
You will find the necessary info right there.

Goto CNET and download Belarc Advisor. it will tell you what the make and model is
