During WWII, the headgear worn by pilots was intended for protection against the cold of high altitude and were not helmets for protection, although USA bomber crews did use infantry type helmets now and then as protection against bumps and sharpnel. The Japanese wore soft head gear, fur lined, against the cold, thus the kamikaze never wore helmets. Flying helmets came with the advent of the jet airplane and I suppose its use is more for storing communication equipment than for protection. The lethal injection swab is truly nonsensical except for the small chance of a last minute reprieve.

With kamikaze this is simple:
Before kamikaze dies he has to GET to where he is going, and also he must KNOW where he is going. The standard ww2 helmet included air and radio. So without the helmet kamikaze would be cut of from communications and might even die if the plane climbs to high.
With needle this is harder -- but one explanation might be this:
What if the governor calls in and pardons the person after the needle is stuck in but before the poison is injected? You would not want a pardoned man suffering from gangrene, do you?

Perhaps it is psychological. Not wearing a helmet would serve as a reminder that they are sacrificing their lives and may induce panic.
As far as swabbing before lethal injection - it is standard procedure. Why change routine just to make a point?

It was not necessarily the last mission; they may be required to commit the act preceded by a spiritual ceremony conducted by a priest with sake, as the object of toasting to the ancestors.
Injections:
The manual of hygiene and procedure unchanged followed to its logical conclusion;

A medical professional "rules" of providing to those on the death bed with fair and available treatment;

They're dumb enough to become kamikaze pilots, they're dumb enough to spend the money on helmets.... My guess is that during the dive bomb down towards the target, things can get pretty bumpy -- don't want the pilots hitting their heads and passing out on the way to the target -- mission NOT complete... Money not well spent...

The pilots' helmets help the pilot to breethe while he is flying in the air.
And the Kamikadzes' goal is reaching the target and damage it. So - they should wear helmets if they wanna hit the target!

Believe it or not, the radio receivers or ear plugs were in those "helmets". They needed to hear any commands on change of direction or target opportunity.
