Yes, you are in deep trouble.
Try running your Anti-virus program from SAFE mode. Use the F8 key during startup. If you have never done this before, hit F8 just before you see the typical Windows Startup screen. You GOT IT when you see the SAFE Mode selection menu.
If this fails, disconnect your internet (if you have high speed), then start making a backup of all of your data in the MY DOCUMENTS folder. Delete any email that might be infected, then use EXPORT from the FILE menu to save your email as a .PST file.
Then get all of your disks together, and install Windows again. Just insert the CD and reboot. Use the DELETE partition option, then reinstall from scratch. This will format the hard drive. This will ensure that you have totally cleaned up the virus.
You have to be much more careful. Always check your anti-virus status, and run scans regularly. You should have an anti-spyware solution on your computer. I recommend that you go to http://www.download.com and get AD-AWARE and SpyBot Search & Destory. Install, run, update and scan your computer with both programs.

Go search for a program called stinger. Mcafee makes this free trojan scanner it scans and cleans at the same time and is free

reboot the whole computer using the windows xp system disk or windows 98 depending what os you are using, or use the system go back on the computer and go back to a day when the computer worked the best
