You don't say what kind of sub-floor you have.
If concrete, it shouldn't have occurred. You may be able to lift it and re-glue. The grout will come up with it, and need re-grouted.

If a "wood strip", same, but it will occur again.
Strip wood expands and contracts.
If strip wood, a suitable underlayment should have been installed.

In any event, it should be repaired best way possible. The tile may crack.

I doubt that you'll be able to match it after 5 years. Like everything, patterns change, aside from the fact the same pattern may not match from a different "run". Almost like "dye lots".

Just had the problem. It cost me big bucks. We had a water leak that slowly drained under the tiles and caused the flooring underneath to rot and pop the nails. Had to tear out the subfloor. You can't fix just the tiles. Get a hammer and tap on all of the tiles around it. Do they sound hollow?

houses sag and settle, boards warp and swell. It happens, its just part of having a house.

You can buy tile repair glue and just reglue it to the floor.

Also, i'd go downstairs and check to water damage, make sure some pipe isnt spraying up onto the subfloor.

You might have to rip up the whole floor and start again because once one comes up they all might start coming up especially if you had this paticular tile for five years.
