What's the correct grammar?

Technically, it should be "if worse comes to worst".
The message you're attempting to convey is that...if a situation that is already bad (worse) deteriorates into a situation that is even "more bad" (worst)...then you should do...etc.

I thought it was: Worse comes to Worst

Plus Billy Joel has a song called "Worse comes to Worst" so I am sure that is correct.
