I played tennis last night and may have aggravated an old high school injury. Last night there was some pain, but this morning I have a pain in my lower back. The difference with this one is that it feels more in the spine than a muscle. I can't stand up straight without it hurting. Should I apply heat or ice? What can I take that won't make me tired?

Sounds like you need to alternate between the heat and ice. 20 minutes of heat, wait a while and then ice it for 20 minutes. Stretching before any activity is the best way to prevent injuries. Motrin is the best for your situation.

Apply ice pack for the first 2 days. After that Hot packs.
Do not play any more till you get better.

Two simple exercises that will fix you up in 24 hours if it is not serious.
1. Lie straight down on the floor... hands by your side... and lift both legs up together up to 90 degrees. Repeat 10 times if possible. If hurts too much stop.

2. Lie on your stomach... hands by your side... and do chest lift... Repeat 10 times.

This is to get the muscles into stretch and relax positions.

Where exactly is the pain? If it is the small of your back, then it is more than likely a herniated disc. Does the pain run down your leg? You need to see an orthopedic doctor. Having a herniated disc is a very serious problem. Heat and/or ice does not help, and no over the counter medication is going to help.

You may want to have a D.O. or CHiropractor evaluate your problem. They are trained to treat this kind of injury.
