I know John Lennon was on the very first cover (1967) Madonna and Prince weren't known, famous pop stars back then... they were kids. I don't think there was a designer per se for the firsts issues.

I know that John Lennon & Yoko was on the first cover - naked, back view. I guest the designer was a staff member that worked at Rolling Stones magazine.
