As far as I know, it's short for Skipper, ( captain of a boat or ship) which is, itself a nickname. Bill and Dick are in a different class of nicknames; they are diminutives or shortings of the formal name.

I dont think there is, i know 2 skips, and their names are John, and Thomas, I pretty sure its just a nickname
