Reach under your cabinet or sink for the shut off valve first -if you let the hot water run it will be the warm one. Remove a cap and a screw usually on most faucet handles. This may be an Allen type(the hexagonal) or a Philips(+) screw. Find a big nut that will hold the valve in place. You will need some big pliers or adjustable wrench to remove this. Pull the valve out carefully and slowly paying attention to anything that might fall off and take this to your local hardware store. Find the replacement valve and reinstall exactly as the old one was removed. Good Luck.

I saw this on This Old House.
There is a screw (either hidden or visible) that needs taken off of the top of the handle. These screws come in various forms, i.e. hex, flat, or phillips. Once off carefully pull the faucet knob off and there should be washers. Goto your local hardware store and find washer(s) of equal size and water repellent lubricant. Replace the washer(s) as you did before and follow the steps backwards to re-fit the faucet knob. Remember to turn off the water prior to starting.

most faucets no longer use washers.. You might think about replacing the entire piece. I had to do that through my whole house, well kitchen and 2 baths.. sure looks nice;-) they never drip

Turn the water off. You did not mention the type, some you need to replace a cartridge. Anyway just take your crescent wrench and take the whole business out, Take it to your hardware store and ask them what you need.
