Talk to a lawyer who specializes in corporate law...generally you incorporated either federally or provincially or by state

You should be able to get guidance from your state government. When you incorporate you incorporate in a specific state, so best to contact that state government for their rules and guidelines.

Get a Federal Tax Id number from irsforms.gov website and go to corporations tab. This will tell you everything you need. The forms are available right there.
