If you have two things that are related, call them x and y. They have a linear relationship if y changes in a way that is proportional to the change in x. SLOPE means the change in y (either positive or negative) when you increase x by 1. Y-INTERCEPT is where we start when there are no x's. One example is that when I make a stack of books on a table, the height of the books (y) changes with the number of books I have in the stack (x). The slope is how much it changes when I add one book (the height of the book) and the y-intercept is the height when there are no books (the height of the table).

The slope of a road grade is based on it's length based on some sort of range finder or map and height of the top and bottom based on the difference compared to sea level.

That depends. For example, in an experiment the compound heated goes 1.2 C up by minute. Then, X axis would be the time, while Y axis would have the temperature. THe slope would be 1.2. I hope this example was useful
