It's normally all subjects combined, but some schools do it differently, for example, I went to a university that threw your hours into the equation. Check with a registrar to see how exactly your school calculates it.

I think most universities do 2 separate GPA's - you should see them on your transcript or "report card". One GPA is the culmination of all courses you've taken. The other GPA is just for your major - all the core classes you've taken that are directly related to your major. Usually, the GPA in your major is more important than your overall GPA.

GPA is all classes.... some techincal degrees will tell you to calculate the classes required for you major seperately!!
