Often, the high priest, if he could manipulate and control the Pharaoh. In many ways, I suspect the chief scribe was very powerful, but frankly, that would be a cheating and manipulative power if it was not consistent with the will of the Pharaoh. I am not sure of the title "grand vizier" and how far back in time it goes. I thought that was more recent than ancient Egypt.

Officially, of course, it was the Pharaoh.

Depends on the Dynasty. In some times it is D, and other times it was A or C.
