Their are three divisions in each league. The team with the best record in each division makes the playoffs, and the team with the next best record of all the non-division-winners (in other words, the best second place team) from each league gets a wild card berth.
So:
American League: 3 division champs, 1 wild card
National League: 3 division champs, 1 wild card
The teams are seeded according to their records, although baseball doesn't explicity number the seeds like other sports. The wild card always has the lowest seed, regardless of record. So the first round matchups are as follows:
Best-record vs. Wild Card team
2nd best-record division winner vs. 3rd best-record division winner
There is also the caveat that two teams from the same division never play each other in the first round. So if the best record and the wild card both come from the same divsion, the wild card will play the 2nd best record division winner.

There are two leagues. Each league has an Eastern, Central, and Western division. The team with the best record in each of the six divisions is ensured a playoff spot.
If a team does not win its division but has the best record of any team in the league that didn't, they are what's called the Wild Card, and are given a playoff spot.
