And then when he wakes up he's back in bed with Suzanne Pleschette in his earlier series and explains away the entire series (multiple seasons) as a dream....

I don't think that people should give away twists because it's great when you find a twist that you aere not expecting.

for movies, it would be Sixth Sense and Derailed. For tv, i would say the season finale of Ghost Whisperer when Andrea didnt know she was dead until the last scene when her brother showed up and hug Melinda instead of her.
