did you see me ever

Not strictly correct, but understandable. Correct would be "Have you ever seen me?"

-do you saw me ever
-did you saw me ever

These are also not correct, but the closest to a meaning they have would be with the verb "to saw", which is to say, "to cut with a saw". These are questions that a magician's assistant might ask the magician. Perhaps a hypnotist/magician, since I would think the person would remember having been sawed or not.

In any case "ever" goes between the subject (you) and the main verb. It usually goes with the present perfect (have/has + Past participle/third form)

All of them understandable, but not correct in english grammar.
for 1, use present perfect tence
for 2, do and saw aren't in the same time.
do is preset, saw is past

for 3, did and saw are past , but the word did make the sentence past, saw must be see.

None of them are sentences. The first is a sentence fragment that might be completed as "...mow the lawn without sweeping the sidewalk afterward?" The second and third cannot be completed in this fashion as the resulting "sentence" would be grammatically incorrect, so it would be misleading to call them sentence fragments.

I agree with zil28, the first answerer. None of the sentences you have given is correct. It should be:

Have you ever seen me ?

All wrong, the correct grammar is to say;
