You should be able to download any US income tax form you need on IRS.gov. I believe that the withholding tax slip is called the w-4. You fill out a w-2 when you get a job, stating how many dependents you have, then at the end of the year your employer issues you 4 copies of the w-4: one for the feds, one for the state, one for other tax jurisdictions, and one for your records
