I understand that one must not address a lady by saying Dear Madam....dear Ms so & so or dear Miss/Mrs so & so is correct.
Why is this??

Dear Madam used to be correct, but then people got it mixed up with the madams who ran houses of ill repute, so they stopped using it.
Dear Ms XXXX is now preferred. For business purposes, it doesn't matter if the woman is married or single, so you don't have to worry about calling her Mrs. or Miss., just plain Ms XXX.

If you don't know the name, Dear Sir or Madam: is completely acceptable.
