There are quad and pentuple bonds. They are not ionic, whether they can be called "covalent" is another matter. If you allow d-orbitals in covalents they are covalent.

I don't think so. I've only heard of single, double, and triple, and only for carbon, oxygen, nitrogen, or sulfur.

no, there will have so much strain that the resulting compound will be highly unstable....
