This is usually caused by either a dirty or scratched disk.
If it is dirty, use a liquid soap and wash it carefully. Rinse and dry by blotting, not rubbing.
If there is a scratch it may be ruined. Wash the disk first and then use a damp soft cloth and carefully rub at the scratch from the center outward.

;-D try using a disk recovery program to get usable data off the disk.
