It has happened. I now carry pen and paper with me most places I go for just such an occasion.

Yes, and I am having trouble coming up with one, I usually ask around 10 questions a day now, got to make it count :).

Yes. It tends to come back - especially if you really need a reality check on that day and/or want to learn something.
