There are programs you can purchase that will do a fairly good job of it but if you clear the history, delete temp internet files, and run a program called clean cache you would clear most of it up...
If you are dealing with a computer on a school or work network there's not much you can do because the system records maintained by the network aren't kept on your computer.. The system admin can pretty much print out your history with a click of a button....

The best way to get it off is using a proxy, I recommend the HideIP Platinum software, this hide your real IP. Also delete all the traces, to do this I use CCleaner.
Another way is making a snapshot of your hard drive before browsing the web (I use Rollback RX)... when you're done, just restore the snapshot and everything you've done will be overwritten, just like if you have never turned on the PC, of course if you want to save your downloaded files you can create a partition that wont be affected by the rollback software (u need to specify this)... whit Rollback you can do it just in a few seconds

The fellas are right, there is always a way. The question is who is going to look? Taking the steps above will keep mom and dad from finding out but not an expert or even a computer literate little brother.

unless you smash the disk with a sledgehammer then bury the pieces in the antarctic there's ALWAYS a way to find out.
it just depends how badly the person wants to find out and what lengths they'll go to to retrieve the info.
if you need to cover your tracks that badly, then i don't wanna know details. obviously your isp has a record of every site you ever visited and every file you ever downloaded regardless of what you wipe off your computer.
