VOLUNTEER AGREEMENT

In signing this agreement, I understand and agree to the following:

I agree to attend a Volunteer orientation session.

I agree to be supervised by the Volunteer Coordinator or a designated staff person, and I will take direction and criticism directly to the Volunteer Coordinator or a staff person.

I will wear a name tag while volunteering.

I will come appropriately dressed, wearing closed-toed shoes and clothing appropriate for my position. I will not were excessively tight, short or revealing clothing.

I will be on time and I will call the Volunteer Coordinator or the shelter if I am going to be late or absent. Two (2) unexcused absences will expel me from the program.

I will not leave my work site during my shift without notifying my supervisor.

I agree not to interfere with any euthanasia decisions and/or procedures.

I will allow the Humane Society to use any photographs taken of me for use in public relations.

I will treat all animals, people and property I come in contact with at the Humane Society with respect. I will refrain from using profanity and I will conduct myself with courtesy at all times.

I agree to maintain confidentiality and refuse to disclose any information that is private or confidential.

I authorize the Humane Society to seek emergency medical treatment for me in case of an accident, injury or illness and to hold the Humane Society harmless in such an event. I understand that as a volunteer I am NOT covered by any worker’s compensation policies of the Humane Society. s

I waive all claims against the Dodge County Humane Society and/or their directors, employees and volunteers for all personal injury and property damages resulting from volunteer work with the Dodge County Humane Society.
