The cartilage is the top part of your ear, for future reference.
But Honestly it hurts about the same.
The cartilage is more prone to infection and hurts to sleep on sometimes, but then again, the double pirecings hurt to sleep on.
Basically it depends on how high your pain tolerance is and weather or not you ahve the time to keep your ears clean.
If you don't have the time, id' get the double. If you've got time to take it out clean it and all that jazz, then go for the cartilage.
But they hurt about the same amount.

Speaking from experience, I have 2 holes in my lobes of each ear, and my tragus, daith, and rook pierced in both ears. I would say cartilage hurts worse, but it really depend on where, because my tragus didn't hurt that badly, and my daith hurt some what, but my rook was very painful, but all of them for just a moment. Most of the pain will come after a piercing from the soreness. All I can say is just close your eyes and breath deeply through the cartilage piercing and you will be just fine. :)

The cartalige hurts more, but I advise you NOT to get two peircings at once on one ear. That is too much! THe ear will swell up between peircings and do not so nice things... It will be more prone to infections and stuff that way. Let each hole heal (on each lobe) before you get another one in the same ear!!! You Will be much happier!

It's about the same. Neither of them really hurt that much, it's so fast, it just feels like a hard pinch with a warm sensation afterwards.
