A stress free day or plan a surprise birthday party for her with a few friends and family or better yet instead of getting her a present take that money and donate it to her favorite charity that means a lot to her. If it was me I would prefer the money to go to cancer research. This would be more meaningful than anything.
The big thing is to make sure you talk it over with your sister-in-law and get her ok. Even talking about this with her will be something cause she will realize that someone cared enough to ask and talk to her .

if she collects anything for her home would be nice. get her a gift certificate to a spa or a massage all women like that!
