Could be many reasons.
They haven't received enough sex education to understand the need for them.
They understand the risks but think they're immortal or indestructible.
They're selfish and only looking out for their own pleasure.
Their partner has not insisted on it.

Men don't stop to think about it for one. If you're sexually active and you know you intend to sleep with someone when you go out, you need to bring one with you and put it on them yourself or tell them it's no deal. Getting pregnant unexpectedly will put a kink in your entire future. Men feel like it's not their responsibility, so it falls on you to take care of your own sexual health and safety.

Because they aren't made to. If you don't insist, then they aren't gonna do it. It's your body...stand up for yourself. If he says no...then he isn't worth the time anyway.
