Do not type your title all in capital letters. Do not put quotations marks before and after the title. Do not underline the title, or put a period at the end of the title. Proper names of people and places as well as important words are capitalized in the title, but prepositions and conjunctions are normally shown in lower case letters, e.g. Harry Potter and the Chamber of Secrets. The same rule applies to headings and subheadings as well.

No. The only time you use either underlining or quotation marks is when you are quoting the name of a play or another title that's not your own. For example, if your title is Symbolism in Romeo and Juliet, you would underline Romeo and Juliet but not the rest of your title.

Best to ask the professor or another student. I've had teachers who wanted the title underlined and then I've had teachers who didn't (I made the mistake of underlining it and received a 5 point deduction), but I've never seen one in quotation marks.

Actually, putting in bold should be fine. I don't believe you have to underline or quote your own title for your own paper.
If you look at books, their titles are not quoted on underlined. Also magazine articles are not quoted or underlined. It's not necessary, only if you're putting the title of another work in your paper like in the works cited(bibliography) or you mention it at some point in your writing.

Neither, you shouldn't underline or put it in quotations. At colleges they usually go by the MLA format and that is to just center it.
