They take the last 1.5 inch of hair closest to the scalp, and it goes back quite a ways. It depends on how fast your hair grows, but months to years is possible. If you shave your head it will be impossible to test, but also pretty obvious.

6 months to a year. According to my friend who has to do drug tests once a month so that he can live with his mom and dad. He says that this is in the new hair growth so do get any ideas about cutting it.
