No you don't need the S. He ought to go. She ought to go. They ought to go. It would be better to use should rather than ought. Example. He should go to the doctor since he is so ill. They should go on the highway rather than the back roads.

Do you want the question form of "I'd better go now?" Usually if you are using that phrase it isn't in a question form. If you want to ask a question like that, it would be: Should I leave now, since you are so busy? It would be correct to say: Shall I leave now, since you are so busy? Most of us do not use shall in every day conversational English though.

You would say "He ought to go." or "She ought to go." You would never say 'oughts'.
The question would be 'Ought I go?", "Ought he go?" or "Ought she go?" but would not be a good way to say what you mean. You would be better saying "Should I go?", "Should she go?" or "Should she go?"
'had better' would be used like "Had I better go now?" as a question.
