"Doctor" is a title.
"Scientist" is a state of mind!
The title "Doctor" is given for different reasons to different people in different cultures. In Portugal, if you get a degree in anything, you have the title "Doctor", and this does not mean much!
In UK, the Doctorate comes from studies. As an MD, you have a doctorate. That does not make you a scientist.
PhDs are also Doctors, but in one field only. The difference is that to get a PhD, you have to "invent" (discover) something that did not exist before, make it public. You have to bring something useful to the World. If not, you do not get a PhD.
To get that PhD doctorate, you have to act as a scientist during your research. It does not mean you will continue to do so for the rest of your life.
All these doctorates ARE titles given for some form of achievement.
The "title" scientist does not exist.
But, since a scientist performs RESEARCH on something, to bring something useful (or not!) to the World, I can say that I know a lot of true scientists that do not even have a degree (or any other title for that matter).
As long as someone is expert in a field, AND continues to SEARCH fot things in that field, he/she IS a scientist.

No scientist would consider a doctor to be a scientist. However, many doctors believe themselves to know everything and may consider themselves scientists. Doctors (internal medicine) are more like artists -in my opinion.
Also, "medical science" doesn't exist. There is scientific research and it may sometimes relate to human health.
And lastly, any PhD is also a "doctor"- of another sort.
