Such a question was asked at the end of the "Petoria" episode. After a little debate, the consensus among my circle of friends is this;

Brian and Chris can hear what he says.
Peter, Lois, and Meg cannot hear what he says.

His other interactions are questionable. (His real world interactions, and not his flashbacks/fantasies).

Obviously, the all speak to (or at) him. But when he responds to them, they don't hear/understand him.

I think the more appropriate question might be "can they UNDERSTAND Stewie?" Brian definitely can, as the two of them have in depth conversations. If you pay attention to how Lois responds to Stewie, it seems like she at least gets the general idea of what Stewie is saying. My theory is that what we hear Stewie saying is how Stewie thinks he sounds. Just like a real toddler is very aware of what they are saying even when it might be hard to understand to an adult, Stewie's words sound more like baby talk to the rest of the Griffins. (Except for Brian of course.)

Yes, they can hear stewie. They just don't really listen to what he says. This is revealed at the begenning of one episode (I forget which one) where Lois is having a dream. She is in Stewie's room, and finds all sorts of weapons. At this point, she says something along the lines of, "I should have listened to what [Stewie] was saying!"

It's kept vague and inconsistent. In fact, at the end of one episode, a group of students in the future are watching the show, and one student raises his hand and asks something along the lines of "What's up with that baby? Can everyone else hear him or what?"
Brian seems to be the only one who can always hear and understand what he's saying. . . along with his radio audience from last night's show.

lol, they can understand what he says. But not exactly He sounds like an evil kind of person, but the family hears him as a baby saying similar things:

eg.Stewie: I hate you and you shall go to hell

Family hears: I don't like u

OK, maybe that doens't sound right, but I'm guessing something similar

Of course they can !They just don't take him seriously with all his death threats. After all he is only a baby!

i believe they can understand what he says, but not his words. meaning, they hear him as baby talk, mumble
