Dear Boss,
Unfortunately I must resign from this place that I lovingly refer to as Hell. I've been offered a position that is in a much better company, for a much better salary, and will no longer be working for the slave wages given by you. I thank you for allowing me the opportunity to work here and endure in the soul-sucking environment with the halfwits you say are my employees, while you continually make me do all your BS work while taking credit for all my ideas.
Good riddance,
Employee X
Oh wait.. that's the honest version...

this was wat i have submitted recently, though my first intent was to throw the letter at her face, shout "I QUIT" and walked away. but bearing in mind the earth is round, never never ever burn my bridge and here came this polite letter.
Dear XXX
As required by my contract of employment, I hereby give you XX notice of my intention to leave my position as XXX.
I have decided that it is time to move on and I have accepted a position elsewhere. This was not an easy decision and took a lot of consideration. However, I am confident that my new role will help me to move towards some of the goals I have for my career.
Please be assured that I will do all I can to assist in the smooth transfer of my responsibilities before leaving.
I would like to thank you for all the guidance and opportunities given.
