I thought Heifers was just another name for cows, but I looked it up and it's a breed of cow. So, when you call someone a Heifer you are basically calling them a cow; that's where the insult comes in!
I've included a link if you want to know more about the cow.

A heifer is a young female cow that has not yet had a calf. This is often used as a remark to indicate overweight but more often it's just a word to describe a woman who's not liked. It can also be used a way to tease a friend who's said something funny to you.

a heifer is a juvenile female cow that has not been bred or given birth, but i wouldn't take it as an insult. they are usually pretty creatures with a lot of energy and vitality. the insult would be to have actually been called 'cow' thus insinuating you had been used and put out to pasture with sagging teets.

A female cow, normally the larger in terms of girth, and slower, because it is female.
