Until I read the "adults only" part I was going to say one dress as a ball and the other as a chain. You probably want something unique to win so I would go with the pirate gig as that will be overplayed with the PotC movie out. If you want sappy I would say she goes as a heart shaped lock and him as the matching key. If you are looking for sexy outfits I am sure a simple web search will turn up a few ideas,i.e. harem girl and sultan, his and her togas, sexy witch and wizard, Hugh Hefner and bunny, pimp and hooker, etc..

two headed person,
Marilyn Monroe and Elvis Presley,
or dress like you're both from any other era, like the 40's.
If you prefer goofy, try a block of cheese and a mouse, or go as a biker couple. For truly scary, go as a zombie nurse and doctor.

I saw this once and it was hilarious... they were dressed as a scottish couple.. the man had the kilt and the whole bit.. but he added a fake member that hung well below the hem of his kilt... and woman also wore a scottish type outfit with excessive cleavage and the kilt tucked into the back with a plaid thong showing.. on her bum she had written.. Kiss me I'm Scottish.... All night long the guy made his fake member swing back and forth between his knees and kept yelling at her.. " Com'ere Lassie.. I have a present fa ya."
