If they're like other states, only the judge who authorizes the search has to sign the actual warrant; however, the judge will require a signed affadavit of probable cause from the investigating officer, which may or may not be attached to the warrant itself. He in turn may have to have a signed affidavit from the witness who is the source of the tip.
