Most of it caused by little razor blades which they use after covering up their faces after a big hit. The good thing about using the blades is that you bleed a lot with minimal damage, if done right. Also, the cuts from the blade heal alarmingly fast, and the wrestler doesn't have to deal with scarring or any serious damage (once again, if done correctly). Sometimes, if the wrestler doesn't have experience with "blading" they can get a stage hand or security guard to do it when they check on them.
Sometimes people are busted up from actual hits (Mick Foley will request this from time to time), but mostly if not done from the blade, bleeding comes accidentally from serious hits or collisions.

There is a lot of blading that takes place, yes...I'd guess that a lot of blood coming out of wrestlers' mouths might be fake (someone with actual internal injuries, like the announcers love to say, isn't likely to get up all that quickly). Additionally, scar tissue often breaks and bleeds quite easily...look at Ric Flair's forehead, then look at how much blood he loses during a particularly nasty match...

Forehead bleeds alot when done with little cuts,,,,plus when it mixes with the sweat...it looks very dramatic. Look at some of the foreheads of the older wrestlers and you will the damage done by the blades they use.

Wrestlers NEVER use blood capsules. EVER. They blade their foreheads. If they're bleeding from anywhere else, it's an actual injury.

They usually have a small razor blade taped inside wrapping on their wrist or fingers and when the time comes they use it to make a cut. It only has to be a tiny cut to bleed and mixed with sweat can look pretty ugly.
