For the real 4 player mahjong, a significant number of the following people play them:
Chinese (especially in HK, Shanghai and Beijing)
Japanese (most REAL 4 player mahjong video games for PS2 use Jap rules)
Vietnamese (not as often)
Phillipino (dragon tiles count as flowers)
Jewish Americans and a few elderly American women (American style, fixed limit hands, NMJL league exists)
Malaysian/Singaporean (they use flowers as well as animals/jokers for bonuses)
Some Westerners around the world (in UK, US, Australia etc) play real 4 player mahjong, but it is less common than poker or bridge, and they usually use Vanilla Western or Chinese Classical rules when scoring the hands.

LOL @Skidoo. That is the most appropriate cartoon ever!!! Im glad they got new partners to replace the dead man. What a relief that must have been for his family!! I remember you saying Faith that the Xbox 360 was not terribly popular yet with you but you hear about this regularly with gamers--playing for days and days until they suffer the inevitable fit! I do find mahjong quite addictive on the computer but not for 32 hours!! xx

I love to play Mahjong on Yahoo Games.A lot of people there ask me how I know how to play since I'm American. It's not hard to learn, and one certainly doesn't have to be Chinese to play it.
