a lot of the what if's were so good that they eventually became integrated into normal continuity. i always like the one about wolverine becoming a horseman of apocalypse, and eventually killing him. if i'm not mistaken, that one came before he actually DID become a horseman. there was a good one about him becoming the head of SHIELD as well. i know i still have them all in a box somewhere, and now i'm gonna need to go read them again to get them off my mind. I BLAME YOU! lol. good question man.

By far the all-time best What If issue was the issue, where all the Watchers got together and watched watchers watching watchers watching watchers. "My name is Uatu. Men call me the Watcher. Women call me 'Peeping Uatu'."
What if Cyclops' power beams came out of his ears instead of his eyes? He'd never be able to take off his ruby-quartz earmuffs!
What if Daredevil was deaf instead of blind? They'd call him The Man Who Can't Hear!"
What if Black Bolt became a rock star? He'd bring down the house!
What if Tony Stark had been an automobile manufacturer instead of an armament manufacturer? Criminals would say. "We're sunk, here comes the invincible Limo Man!"
What if Galactus chose Aunt May as his herald? She'd be the Golden Oldie! And instead of eating planets, she'd make him have Twinkies and milk!
These are just a few samples from that priceless issue, stories that we will never see fleshed out into full-fledged series, because the ideas are just too daring, innovative and arte-nouveau to really publish, so that they had to pretend they were being comical. They would be much more challenging to the writers and to the continuity than the kind of stuff they are churning out these days.

*sigh* my fav 'what if?' is the one with Nightcrawler and Rogue. I forget the title and I can't look in my collection, since I lost all 500 comics when my house burned down last January :(
