It's cultural. The longer you do it, the better it is. There are cultures where it is accepted until 5-6. (Of course with supplementation)

Typically 2 is the best cut off for "extended" breastfeeding. After that the child starts to learn that it's not for nourishment but for comfort and routine. It can cause that child to not develop social skills as soon, etc. I would defeintly stop after 2.

I am okay with it as long as it is done respectfully...
The following is not okay: I once saw a 20 something woman wearing a tube top at a restaurant. Whenever her 2-3 yr old got bored he would come over and pull down her shirt and attach himself. It was disgusting!
I think at this age it should be done privately or with more subtlety.

I think it depends on the Mom. I was ready to have my body back at 12 months so I weaned both my kids. I did it as long as I could and enjoyed it. Some Moms are not ready to give up the closeness...but I never felt I was giving up anything.

Maybe until 2 or 3 years old, but any longer than that and it's just unhealthy.

It depends on HOW extended. A year, maybe. Five years, no.

It would depend on how old the child was. I once heard of this woman who still breastfed her 7 yr.old son. I found it quite repulsing.
