Yes they do. I have had it happen to my betta fish. I was catching in the net to put him in his cup while I changed the water in hiw little aquarium; he darted and I wasn't expecting him to so I didn't move the net and he scraped off several rows of scales on his back. I then carefully removed him and changed his water and carefully replaced him into his bowl. After about 2 weeks (give or take a few days) the scales he had scraped off were back. They are a lighter color and you can see clearly were they are so I guess that fish not only grow back scales but they scar as well. No worries though. I kept my fishes tank clean and he is still happy and healthy with all of his scales.

Yes. As long as the water quality is good and the fish are eating a quality food, the scales will grow back. The new scale might be a little smaller or different color, however.

Yes - the scales fall off as the fish grows. New ones replace the old ones.

Really does depend on the type of fish. In the Rainbow Fish books Rainbow didn't grow back scales.
