It's kept ambiguous on purpose. But a common fan theory is it's Marcellus' soul. The bandage you see on the back of his neck is supposedly where his soul was removed. As to what was in the ACTUAL suitcase used for filming the movie? A simple lightbulb.

There are some clues for that; the bandage in Marcellus neck, the fact that everyone who opens the briefcase see some light and is astonished at that sight, etc
Supposedly, Marcellus had sold his soul to the devil and he was trying to recover it.
